How to Apply

The admission to a community hospital for inpatient care is arranged through referrals from your doctor in the public and private acute hospitals. The community hospital will then update you on the transfer. 

Prior to the admission, the community hospital will conduct means-testing to determine if the patient is eligible for subsidies. If the patient qualifies, subsidies are available from Medifund, MediShield Life, and ElderShield to assist you to defray the cost of staying in a community hospital.
