You can check the status of your application to remove your appointment as a Trusted Contact via your Activities page using your Singpass.

Upon successful submission of the Remove yourself from your loved ones’ Trusted Contact list e-form, your name will be removed from the “Trusted Contacts” section under your Account settings.
