HAS clinics participating in this programme will be auto-enrolled as offering the HPV2 on their appointment system in order to reduce administrative work. There is an option to “opt out” of offering the HPV2 on the HAS Admin portal.

Note: Offering HPV2 on HAS does not automatically qualify your clinic for the Temasek Foundation HPV Immunisation Programme to offer HPV2 at no cost. You will need to separately register by submitting this form.

CHAS GP Clinics that are already on this programme do not have to register again.
