How do I change my child’s race if the other parent has passed away or is uncontactable?

In the case where one parent had passed away, the other parent may submit an application to change the child’s race here, with a copy of the other parent's death certificate.

The parent (applicant) who is applying for the race change is required to declare the following:
- The other parent/ legal guardian(s) is no longer alive and has not appointed any legal guardian in respect of the above-listed minor. 

In the case where one parent is uncontactable, an Order of Court empowering the other parent to change the child’s race unilaterally will be required.
