A copy of the latest Central Provident Fund (CPF) Statement of Account of all the Beneficiaries; if unavailable, to provide a copy of the latest Notice of Assessment from IRAS; and
Supporting documents showing that the Beneficiaries are facing financial hardship e.g. letters indicating that they are recipients of ComCare or Public Assistance Scheme.
