How do I edit my Advance Care Planning (ACP)?
To amend the ACP draft, you will need to re-do it using the LPA-ACP tool on My Legacy - https://go.gov.sg/mylegacy-lpa-acp. Alternatively, you can inform the ACP Facilitator of the changes which you want to make to the draft during the facilitation session.
