We wish to inform you that the mobile clinics are open to only Singaporeans aged 50 years and above with a valid CHAS Blue/Orange card. If you do not meet the eligibility criteria above or wish to complete your LPA and ACP sooner, you can do so by contacting an LPA Certificate Issuer and ACP Facilitator. You can refer to this page - https://mylegacy.life.gov.sg/find-a-service/ to find a service provider.

Do check with your preferred service provider on their charges. For ACP, if you are receiving medical care at a public healthcare institution, you can ask your care team to arrange an ACP session for you. If you are not receiving treatment at any public healthcare institution, you can contact a community ACP facilitator using the link above.
