What are the supporting documents to be submitted for an application of a Massage Establishment licence?

The following supporting documents are required for a Massage Establishment licence application:

a) Coloured photographs showing the following:
i. full front view of staff uniform (with sleeves for top and pants for bottom)
ii. side view of the staff uniform (with sleeves for top and pants for bottom)
iii. rear view of the staff uniform (with sleeves for top and pants for bottom)
iv. company or establishment name or company nametag on staff uniform
v. a close-up shot of the company nametag (if nametag is used)

b) Layout plan of the establishment drawn to scale, stating the total floor area in Square Metres

c) URA planning permission for proposed use of premises to be used as ‘Massage Establishment’ / HDB approval letter (whichever is applicable)

d) ACRA Business Profile
