We strongly encourage you to submit your medical examination form online via our e-Services portal here. However, motorists who are unable to submit the form online can still mail the form or drop off the form in-person at TP Headquarters (10 Ubi Avenue 3), or use the self-service terminals at the TP Headquarters, where TP staff will be available to provide further guidance.
