Please undergo the medical examination and submit the completed medical examination form online via our e-Services portal here or the Police@SG mobile application to reinstate your licence. If your licence has been invalidated for at least 3 years, it will be permanently revoked. You will need to retake your theory test and practical driving test if you wish to get a new licence. Motorists who are unable to submit the form online can mail the form, drop off the form in-person at TP Headquarters (10 Ubi Avenue 3), or use the self-service terminals at the TP Headquarters where TP staff will be available to provide further guidance where necessary
